[Bp_ipv6] Survey citation format

Greetings all,

I think that the outcome document might be more appealing to the reader if
there is a uniform citation format applied throughout. I wanted to start
with citing survey results, and thought that this would be a helpful format:

Survey response, [name], [organization], [country].

Or, in the case of anonymous submissions,

Survey response, [country].

What do people think?
